New Customers: account name, bill to and ship to address, purchase order number if applicable, telephone and fax number, and a description of the products you wish to order.

All Customers: please refer to the ORDER CHART AND SIZING INFORMATION on the page of your selection. We need the name of the product, the size, the material (if made in more than one material), and color (if we offer a choice).

SHIPPING

All shipments are UPS Ground Service unless otherwise requested. Shipping, special handling and insurance charges are not included in our low professional price and will be invoiced separately.

PAYMENT TERMS

Net 30 from date of invoice. Past due accounts will be assessed interest at 1 ½ % per month.

RETURNS POLICY

Returns are accepted within 30 days of delivery. Please include one copy of your invoice with return. Size exchanges accepted when returned merchandise is AS NEW. Returns may be assessed a 20% RESTOCKING FEE.

LIMITED WARRANTY

Palumbo Orthopaedics warrants our products to be free from defects and workmanship for 30 days from the date of delivery. This warranty is in lieu of any other warranty, expressed or implied.
